Cervical stimulation targets the neck. Auricular stimulation targets the outer ear. Both are marketed under the same broad "vagus nerve stimulation" heading, which implies they are simply two routes to the same result.
A 2025 study published in the American Journal of Physiology compared acute cervical and auricular stimulation against sham. Cervical stimulation increased alpha wave brain activity, associated with a calmer mental state, and lowered arterial blood pressure. Auricular stimulation did not produce the same effects in that comparison.
That study also reported decreases in heart rate and increases in heart rate variability in the stimulation group, with the sham group showing neither [6].
Keep this in proportion
The finding came from a single session in a small sample, so it is preliminary rather than settled, and no consumer device is a treatment for a diagnosed condition. But if you are choosing between placements and want the option with the more direct supporting evidence behind its specific approach, the neck-worn category currently has the stronger case.

If your goal is sleep
